Description :

.Alprazolam is a triazolo analog of the 1,4-benzodiazepine class of CNS active compounds It is an anxiolytIc with hypnotic and anticonvulsive properties. Alprazolam presumably produce its effects by binding at stereo-specific receptors at several sites within the CNS. Their exact mechanism of action is unknown. Like all benzodiazepines, It causes a dose related CNS depressant activity varying from mild impairment of task performance to hypnosis.

Composition :

Xanax 0.25   : Each tablet contains Alprazolam USP 0.25 mg.

Xanax 0.5     : Each tablet contains Alprazolam USP 0.5 mg.

Indications :

Alprazolam is indicated for the management of anxiety disorder or short term relief of symptoms of anxiety, anxiety associated with depression. It is also indicated for the treatment of panic disorder with or without agoraphobia.

Dosage and Administration

Anxiety disorders and transient symptoms of anxiety - treatment for patients with anxiety should be initiated with a dose of 0.25 mg to 0.50 mg given 3 times daily which may be Increased to achieve maximum therapeutic effects at intervals of 3-4 days, to a maximum daily dose of 4 mg. The lowest possible effective dose should be employed and the need for continued treatment reassessed frequently. The risk of dependence may increase with dose & duration of treatment. Panic disorder- because the management of panic disorder often requires the use of average daily dose of Alprazolam above 4 mg, the risk of dependence among panic disorder may be higher than that among those treated for less severe anxiety. In elderly patients with advance liver disease, the usual starting dose is 0.25 mg, given 2-3 times daily and dose may be increased gradually if needed & tolerated.

Side Effects :The side effects are generally observed at the beginning of therapy & usually disappears upon continued medications. In the usual patient, the most frequent side effects are drowsiness and light-headedness. Besides, depression, headache, confusion, insomnia, dizziness, nervousness, dry mouth, constipation, diarrhea, nausea, vomiting, tachycardia, palpitation, blurred vision, weight gain, weight loss, increased salivation.

Contraindications : It is contraindicated in patients with known sensitivity to this drug or other benzodiazepines and active narrow angle glaucoma. May be given to patients with open angle glaucoma who are on appropriate therapy.

Precautions : It is recommended that the dose should be limited to the smallest effective dose to preclude the development of ataxia or oversedation which may be a particular problem in elderly or debilitated patients. The precautions in treating patients with impaired renal, hepatic and pulmonary function should be observed. There have been rare reports of death in patient's severe pulmonary disease shortly after the Initiation of treatment with Alprazolam. Decreased systemic Alprazolam elimination role has been reported In both alcoholic liver disease patients and obese patients receiving Alprazolam. Alcohol should not be used during treatment with Alprazolam. During the course of treatment a patient should not either drive vehicle or operate machinery. Dose should not be increased without consulting physician. Alprazolam should not be stopped abruptly or decrease the dose without consulting physician, since withdrawal symptoms can occur. The use of Alprazolam at dose greater than 4 mg/day often necessary to treat panic disorder, is accompanied by risk that need careful consideration.

Use in Pregnancy & Lactation :

Pregnancy: Alprazolam has been categorized In pregnancy category "D" that means, it should be avoided in pregnancy.

Lactation : Like other benzodiazepines, Alprazolam is assumed to be excreted in breast milk, Therefore, nursing should not be undertaken by mothers who must use Alprazolam.

Warning :

Certain adverse clinical events, some life-threatening are a direct consequence of physical dependence to Alprazolam. These Include withdrawal symptoms, the most important is seizure. Even after a short term use of the dose recommended for the treatment of anxiety and anxiety disorder (i.e.0.75mg-4mg/day), there is some risk of dependence, early morning anxiety & emergence of anxiety symptoms between doses of Alprazotam have been reported in patients with panic disorder, taking prescribed maintenance doses of Alprazolam, which reflect the development of tolerance or a time interval between doses which last longer than the duration of clinical action of administered dose, withdrawal reactions may occur when dose reduction occurs for any reason. Safety & effectiveness of Alprazolam is below 18 years have not been estabilsned.

Drug Interaction :

Alprazolam produces additive CNS depressant effects when co-administered with other psychotropic medications, anticonvulsants, antihistamines, ethanol & other drugs which themselves produce CNS depression. Steady, state plasma concentration of lmipramine and desipramine have been reported to be Increased an average of 31% & 20% respectively by the concomitant administration of Alprazolam.
